Domino’s Pizza, 11442 West Broad Street – The following violations were reported during a routine inspection: racks in bottom portion of pizza make table and cheese rack in walk-in are rusty; lowboy at pizza make table is dripping water; walk-in fan guard, black-coated pipe on walk-in ceiling, and front vent of the pizza make table are dusty; floor tiles are cracked in front of pizza oven and along the back hall.

Hampton Inn & Suites Glenside, 5406 Glenside Drive – The following violations were reported during a routine inspection: hand sink doesn’t have hand soap; observed an employee fail to sanitize utensil and plate after washing.

Residence Inn, 5416 Glenside Drive – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; observed pink/orange residue on shield inside ice machine; cutting boards stored in the equipment closet are heavily scratched.

Jersey Mike’s Subs, 7000 Forest Avenue – No violations reported during a follow-up inspection.

Einstein Brothers Bagels, 5607 West Broad Street – No violations reported during a follow-up inspection.

Mosaic Restaurant, 6229-A River Road – The following violations were reported during a follow-up inspection: air temperatures of mussel drawer, salad table and mid make table are elevated; no thermometer found in mussel drawer or mid make table.

Hai Ky Mi Gia, 1601 Willow Lawn Drive – The following violations were reported during a follow-up inspection: slicer is dirty because it was not disassembled to clean and sanitize; temperatures of ground pork, tofu, bean sprouts and brisket are elevated; pho is not date marked.

Mezeh Mediterranean Grill, 11710 West Broad Street – The following violations were reported during a routine inspection: facility doesn’t have written procedures for cleaning up vomit; observed buildup around nozzles of juice/tea dispensers; temperatures of spinach, tabbouleh and hummus on the line are elevated because the containers are overfilled; foods in the reach-in incorrectly cooling with both plastic wrap and a lid; containers on dish rack are stacked while still wet; single-service bowls on the line are not stored inverted; gasket on lowboy across from the line is badly torn; door handles on same lowboy are missing/broken; center line racks under the table are very rusty; one lowboy door under the dough station doesn’t close due to an ill-fitting gasket (facility is aware and no food is stored there); contact paper on shelving is peeling; walk-in cooler racks are dirty; observed a dark buildup on lowboy freezer gasket; observed low grout in various spots throughout the kitchen; floor under walk-in racks has patches of soils; wall in dish area has mildew.

El Torito Mexican Restaurant and Grill, 5600 Williamsburg Road – The following violations were reported during a routine inspection: observed the cook vaping on the cook line; bar hand sink doesn’t have hand soap; lemon found in hand sink in service area and raw meat is thawing in kitchen hand sink (hand sinks are for handwashing only); temperature of refried beans is too low; temperatures of chicken wings, tamales, fries and queso fresco in the glass door reach-in are elevated; this unit needs to be repaired; raw meat incorrectly thawing in standing water; large plastic bin is stacked while still wet; ice bucket is not stored inverted.

Fruit Bowl LLC, 2101 Maywill Street – The following violations were reported during a routine inspection: dish area hand sink doesn’t have paper towels; spray bottle of cleaner is unlabeled; facility bottles cold pressed juice and the labels do not contain a warning or other requirements.

Live Crawfish & Seafood, 9020 West Broad Street – The following violations were reported during a follow-up inspection: temperatures of broth with crawfish and celery, cooked crab and cooked crawfish all made last night are elevated because they weren’t cooled properly; didn’t observe a hanging thermometer in the reach-in units; multiple boxes of shortening incorrectly stored on the floor; spoons and forks are not stored inverted; multiple units in the kitchen have torn gaskets; cutting boards at three-compartment sink are heavily scratched; observed debris on racks throughout facility; observed grease on hood filters and light bulb covers on hood system in kitchen; walls near hand sink are stained; observed debris on floor under dry storage area in kitchen.

American Pizza and Wings, 5401 Glenside Drive – The following violations were reported during a routine inspection: observed dried food debris on slicers and dried grease on tongs; temperature of rice that had been reheated is too low; temperatures of cheese, wings, breading and egg wash are elevated; no date marks found on ham, bologna, turkey or an open sauce can in the walk-in cooler; raw fish incorrectly thawing in reduced-oxygen packaging; menu lists catfish but only swai was found in the cooler and freezer; there isn’t a designated area to store heavily dented cans to be returned; observed dried food debris on bottom shelf and inside doors of sandwich unit and outside trash can in dish area.

Quality Inn Richmond Airport, 5240 Airport Square Lane – The following violations were reported during a routine inspection: observed one dead cockroach on the floor in the kitchen; glass door mini cooler on the breakfast line is not keeping foods at the proper temperature; facility couldn’t provide test strips for quat sanitizer.

International House of Pancakes, 4840 South Laburnum Avenue – The following violations were reported during a routine inspection: cooked bacon is not at the correct temperature; hashbrowns are kept on time control but there isn’t a written policy; eggs cooked “sunny side up” and burgers are not connected to the consumer advisory on the horizontal menu; powdered sugar shakers are not labeled; mop sink doesn’t have a dual check valve installed; observed grease and debris on and around old grease receptacle outdoors; side door of outdoor dumpster shouldn’t be open.

Pho So 1, 9135 West Broad Street – The following violations were reported during a routine inspection: person in charge is not ensuring that employees are rapidly cooling foods not held hot or not for consumption within four hours; person in charge is not ensuring that employees are properly maintaining the temperatures of foods being held hot or cold; didn’t observe a certified food protection manager certificate; kitchen hand sink doesn’t have paper towels; raw chicken incorrectly stored over raw beef in lowboy unit; raw beef incorrectly stored next to green onions and cooked pork bones in reach-in unit; observed food debris greater than one day old on knife in container, inside mixing bowl and on stored ladle; temperature of rice made last night is elevated because it was bagged and placed in a deep container; temperatures of pho broth, cooked pork, noodles and sauce with garlic and oil are too low; temperature of raw beef in prep top unit is elevated; no date marks found on multiple soups in lowboy unit or on pork cake made two days prior; temperatures of lettuce, shrimp, sprouts, egg rolls, spring rolls, noodles, tapioca and cooked pork are elevated because they are stored out at room temperature; Tylenol and Vaseline incorrectly stored next to plates and single-service cups on shelf; Windex incorrectly stored next to soybean sauce in kitchen; dish detergent incorrectly stored next to squash/pumpkin on shelf under prep table; two chemical bottles at service station and on shelf in storage area are unlabeled; improper cooling methods observed for sliced beef, noodles and sprouts; multiple containers of white powders are unlabeled; observed one live roach crawling inside cabinet under prep line; observed two dead insects on shelves under prep line/behind sandwich units; multiple boxes of food incorrectly stored on the floor in walk-in freezer; ice scoop is touching ice in ice machine; rice scoop and spoons incorrectly stored in standing water; metal brush used to marinate raw meats incorrectly buried in sauce; to-go trays are not stored inverted; multiple cutting boards on prep line and at three-compartment sink are scratched; observed debris on shelf under grill, on hood filters, in between and on sides of equipment on cook line, on shelves and racks throughout facility, on floor under equipment on cook line, under equipment on prep line, in walk-in freezer, and under ice machine near service station; baseboard tiles along baseboard in dish room are coming off; low grouting found on floor in dish area and on cook line; walls near three-compartment sink are stained; employees’ personal items incorrectly stored throughout prep table/prep line.
