Marion County restaurant inspections - critical violations Sept. 1-14

The following restaurant inspections with critical violations were conducted by Marion Public Health between Sept. 1 and Sept. 14:

● Primrose Retirement Community of Marion, 1550 Wellness Drive, Marion, Sept. 1. Person in charge (PIC) unable to demonstrate knowledge of cleaning and sanitizing (critical). PIC was unable to say what kind of sanitizing the dishwashing machine does nor could PIC explain how to test dishwasher for correct sanitization levels. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical). Did not observe dishwashing machine to be dispensing sanitizer during the time of inspection. Instructed PIC to use three bay sink until dishwashing machine is fixed. Improper storage of poisonous or toxic materials (critical). Observed chemical products to be stored with food products in storage room. Working containers of poisonous or toxic materials not properly labeled (critical). Did not observe working chemical container to have a common name on it during the time of inspection.

● Ralphie's, 1833 Marion-Mt. Gilead Road, Marion, Sept. 1. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed eggs to be stored above beef in cooler next to stand-up freezer in kitchen. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Observed TCS food in the cooler next to the fryer to not have been properly date marked after 24 hours. PIC placed date mark on item.

● Adkins Dairy Queen Inc., 561 N. Main St,, Marion, Sept. 1. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed milk in cooler with no lid on it. PIC removed container. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed food utensil hung up to dry to be dirty. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed ice machine to have debris on the food contact portion. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed everything sauce and re-hydrated onions being held at room temperature with no time as a public health control stamp or procedure in place. PIC disposed of both items and discussed that facility would begin keeping these items held at 41 degrees or lower rather than keeping them at room temperature. Ready-to-eat, TCS food not properly discarded when required (critical repeat, corrected during inspection). Observed container with everything sauce to have a date mark of Aug. 22, thus being held past its disposition date. PIC disposed of it during the time of inspection.

● House of Hunan, 1583 Marion-Waldo Road, Marion, Sept. 6. Food package(s) received in poor condition (critical, corrected during inspection). Observed two dented cans intended for use on the counter. PIC removed them to be discarded. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed raw meat being stored above ready-to-eat food in the walk-in cooler. PIC arranged containers so they were stored properly. Working containers of poisonous or toxic materials not properly labeled (critical, corrected during inspection). Observed common name of chemical detergent to not be labeled on the bottle.

● Long John Silver's #70045, 1560 Marion-Mt. Gilead Road, Marion, Sept. 6. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical). Did not observe signed sick policies in the facility during the time of inspection. Correct by Sept. 13. Ready-to-eat, TCS food not properly discarded when required (critical, corrected during inspection). Observed raw chicken, cheese sauce, rice and coleslaw to be held past their disposition date. PIC disposed of items.

● Payne's Pizza, 1624 Marion-Mt. Gilead Road, Marion, Sept. 7. Food not properly protected from contamination by separation, packaging, and segregation (critical, corrected during inspection). Observed sliced tomatoes in container with a damaged lid.

● Steve's Dakota Grill, 1677 Marion-Mt. Gilead Road Suite 100, Marion, Sept. 7. Food not properly protected from contamination by separation, packaging, and segregation (critical, corrected during inspection). Observed several food items in coolers to not have lids. Improper cooling of TCS food (critical, corrected during inspection). Observed large soup container to be cooled improperly during the time of inspection. Since it had not yet been two hours, soup was re-heated to 165 then cooled properly.

● Steak 'N Shake #430, 2081 Marion-Mt. Gilead Road, Marion, Sept. 9. Ready-to-eat, TCS food not properly discarded when required (critical, corrected during inspection). Observed sauce being held past its disposition date. PIC disposed of sauce.

● DB's Rock'n Wing Truck, 5425 Stauffer Road, Morral, Sept. 12. Handwashing sink not accessible (critical, corrected during inspection). Items in hand sink; couldn't wash hands during the time of inspection.

● Bowstring Bar & Grill, 109 N. Water St., Caledonia, Sept. 13. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed seasoning pan with no lid on it during inspection.

● Clase All Occasions Catering LLC, 6989 Waldo-Delaware Road, Waldo, Sept. 13. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical repeat, corrected during inspection). Observed chilli sauce in the walk-in cooler to not have a date on it. Working containers of poisonous or toxic materials not properly labeled (critical, corrected during inspection). Observed several chemical containers to not have the common name of chemical(s) on them during the time of inspection.

● G & R Tavern, 103 S. Marion St., Waldo, Sept. 13. Handwashing sink is being used for purposes other than handwashing (critical, corrected during inspection). Observed ice in the front handwashing sink during the time of inspection. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed hamburgers in the cooler to not have a lid on them. PIC threw hamburgers away.

● Tri-Rivers Restaurant, 2222 Marion-Mt. Gilead Road, Marion, Sept. 14. Food in contact with an unclean surface or linens (critical, corrected during inspection). Observed several scoop handles to be stored in seasonings and flower during the time of inspection. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Observed milk to not be date marked after the 24 hour time period.
